ABSTRACT

Objective To evaluate the efficiency of thyroid ultrasound grading formulated by American Thyroid Association (ATA) and fine-needle aspiration (FNA) biopsy in diagnosing benign and malignant thyroid nodules.Methods A retrospective analysis of 357 thyroid nodules in 342 patients underwent surgical excision or FNA biopsy after thyroid ultrasound was performed.All ultrasonograms were graded according to ATA diagnosis guideline for thyroid nodule versi on 2015.The diagnostic efficiency of ATA grading and FNA biopsy were evaluated comparing with postoperative pathological results.Results There were 248 of 357 thyroid nodules confirmed with postoperative pathology,including 233 malignant and 15 benign nodules.The diagnostic accuracy,sensitivity,specificity,positive and negative predictive value of ATA guideline in diagnosis of malignant thyroid nodules was 88.31% (219/248),90.99% (213/233),46.67% (7/15),96.36% (212/220)and 25.00% (7/28),respectively,while of FNA biopsy was 98.81% (83/84),100% (75/75),88.89 % (8/9),98.68% (75/76) and 100% (8/8),respectively.The area under the curve (AUC) of ROC was 0.653 and 0.944 for ATA grading and FNA biopsy,respectively (Z=2.397,P=0.017).Conclusion ATA guideline has high diagnostic value in differential diagnosis of thyroid nodules,while FNA may be more effective in diagnosis of thyroid nodules.

ABSTRACT

ObjectiveTo investigate the clinical efficacy of continual needle knife treatment for cervical spondylosis and whether it increases adverse reactions and the number of treatments.MethodNinety-two patients with cervical spondylosis were randomly allocated to a treatment group of 47 cases and a control group of 45 cases. The treatment group received continual needle knife therapy once every day and the control group, interrupted needle knifetherapy once 10 days.ResultThere were no statistically significant differences in the number of treatments, the total efficacy rate and postoperative adverse reactions between thetwo groups (P>0.05). There were statistically significant differences inthe treatment period and cure rate between the two groups (P<0.05). The treatment period was significantly shorter in the treatment group than in the control group(P<0.05). The cure rate was significantly higher in the treatment group than in the controlgroup(P<0.05).ConclusionContinual needle knife therapy cannot increase postoperative adverse reactions and the number of treatments. It can significantly shorten the treatment period, avoid another injury and increase the cure rate in the patients.

ABSTRACT

<p><b>OBJECTIVE</b>To assess the values of color Doppler and magnetic resonance imaging (MRI) in the identification of cervical lymph node micrometastasis of tongue cancer.</p><p><b>METHODS</b>Totally 96 cases of tongue cancer with impalpable neck lymph node was examined with color Doppler and MRI within one week before surgery. Chi-square test was used to assess if the presence of regional lymph node micrometastasis, histopathological analysis as a golden standard lymph node micrometastasis.</p><p><b>RESULTS</b>For the diagnosis of cervical lymph node micrometastasis, color Doppler was significantly better than MRI in sensitivity (72.5% vs 50.0%, P = 0.039) and the accuracy (78.1% vs 64.6%, P = 0.038), but no significant difference in the specificity (82.1% vs 75.0%, P = 0.357), the positive predictive value (74.4% vs 58.8%, P = 0.159) and the negative predictive value (80.7% vs 67.7%, P = 0.108).</p><p><b>CONCLUSION</b>Color Doppler is better than MRI in the sensitivity and accuracy for the diagnosis of cervical lymph node micrometastasis of tongue cancer.</p>

ABSTRACT

Objective To compare the contrast enhancement effects of SonoVue and Sonazoid in rabbit liver at ultrasonic frequency of 7 MHz using various mechanical index(MI) and dosage. Methods Eight rabbits were bolus injected with different contrast agents:for SonoVue(0. 06 ml/kg,0. 12 ml/kg) ,MI ranging 0. 10 - 0. 55; while Sonazoid (0. 045 ml/kg, 0. 09 ml/kg) , MI ranging 0. 19 - 0. 98. Quantitative analysis of ultrasonography was done using video-intensity analysis system, and peak intensity (PI) was compared. Results With the same frequency, in SonoVue group, for both dosage groups, PI reached maximum at MI = 0. 15 ( P <0. 05) ; with the same MI, PI of the high dosage group was higher ( P < 0. 05). In Sonazoid group,for both dosage groups,PI reached maximum at MI = 0. 40( P <0. 05) ; with the same MI,PI of the high dose group was higher ( P <0. 05). Conclusions The optimistic ultrasonographic conditions provided to SonoVue and Sonazoid are different. Dosage of contrast agents, MI and frequency need to be concerned.

ABSTRACT

Objective To explore the relationships between the quantitative parameters for contrastenhanced ultrasonography(CEUS) in hepatocellular carcinoma (HCC) and arteries in neoangiogenesis.Methods One hundred and fifteen HCCs in CEUS were analyzed off-line with dynamic vascular curve of ImageArena and immunohistochemical stained in the tissue slices.Parameters including maximum of intensity(IMAX),rise time(RT),time to peak(TTP),mean trasit time corresponding to the center of gravity(mTT) ,rise slope(RS) and washout time(WT) were analyzed by statistics.Unpaired arteries(UAs)of 82 HCC cases were succesfully stained.Results The number of UAs had moderate correlation with RT (r = - 0.446),TTP (r = - 0.432) and RS (r = 0.431) (P < 0.05),and it had mild correlation with IMAX (r = 0.303) ,WT (r = 0.285) (P <0.05).Conclusions Neoangiogenesis is a functional adaptation to altered vascular dynamics.Quantitative parameters of CEUS,reflecting hemodynamics of tumors,are correlated with UAs.

ABSTRACT

Objective To study the effect of cirrhosic liver on dynamic vascular curves analysis f(t)of hepatocellular carcinomas(HCCs) at contrast-enhanced ultrasound by exploring the difference between perfusion parameters of HCCs in cirrhotic liver and that in noncirrhotic liver. Methods One hundred and fifteen HCCs were analyzed off-line with dynamic vascular curve f(t) of Sonoliver. The HCCs were divided into 2 groups based on their background liver as cirrhotic liver group ( n = 86) and noncirrhotic liver group ( n =29). Such parameters as maximum of intensity(IMAX) ,rise time (RT,from 10% to 90% of IMAX),time to peak(TTP) ,mean trasit time(mTT) ,rise slope(RS) and washout time(WT) of the two groups were analyzed by statistics. Results There was no significant difference in the f(t) parameters of HCCs between cirrhotic liver group and noncirrhotic group(P >0. 05) [IMAX:(124. 25 ± 5. 56)% vs (113. 65 ± 5. 58)% ; RT: (12. 98 ± 0. 48)s vs (13. 68 ± 0. 92)s;TTP:(13. 70 ± 0. 52)s vs (14. 39 ± 0. 9)s;mTTi(138. 61 ± 9. 20)s vs (149. 49 ± 21. 70)s;RS:10. 11 ± 0. 60 vs 8. 89 ± 0. 77, WT: (34.17 ± 2. 91 )s vs (28. 36 ± 3. 25)s]. There was also no significant statistical difference in all above parameters in reference regions for the two group (P > 0. 05). Conclusions The f(t) parameters in HCCs can evaluate blood perfusion of the tumors, and cirrhotic liver does not affect the perfusion parameters of HCCs.
